The insert was a surprise. The down and feathers were covered with a fabric that is densely woven such that not one feather would be able to work its way through. It is not overly stuffed, so if you're expecting lofty, this is not that. It is firmly packed, however- not loose in the casing. I bought an ivory boucle zippered cover with a one inch difference in width and length, and that stuffed the pillow up more. It's just the right firm support for the lower back, yet soft enough in density to be just perfect for a sleepy head.
